- A list of all medications you are currently taking, including dosages.

- Referral letters, if applicable.
Patient Registration and Check-In
- New Patients: Please arrive 30 minutes prior to your scheduled appointment to complete the registration process. You can download and fill out the registration forms from our website to expedite this process.
- Returning Patients: Check-in at the reception upon arrival. If your personal or insurance details have changed since your last visit, please inform the receptionist.
